The feasibility, acceptability and cost-effectiveness of nurse-led models of chronic disease management in general practice (2007-2009)
Abstract
Workforce shortages, the growing burden of chronic disease and an ageing population are major challenges facing primary care in Australia. To address this challenge, this project aims to test a practice nurse-led model of chronic disease management to patients in general practice. The findings will be significant in addressing gaps in the current body of knowledge about alternate models of primary health care delivery in Australia. Primary outcomes of the study are, the acceptability of this model to patients, doctors and nurses, its cost-effectiveness, the impact it has on health outcomes and related quality of life of patients in the nurse-led arm and how the model addresses the systemic impediments of the current primary care workforce.
