Overview

Background

Dr Paul Williams is a Research Fellow in the Antimicrobial Optimisation Group within the Frazer Institute at The University of Queensland. With more than a decade of experience as an intensive care pharmacist, his research focuses on optimising medicine dosing in critically ill patients through pharmacokinetic/pharmacodynamic (PK/PD) principles, precision dosing, therapeutic drug monitoring, and personalised medicine approaches.

Critically ill patients often experience substantial changes in how medicines are processed by the body, meaning standard dosing approaches may not achieve optimal drug exposures. Paul's research aims to better understand these changes and develop strategies that support safe, effective, and individualised therapy for patients with severe illness.

His research spans antimicrobial optimisation, critical care pharmacotherapy, and clinical dosing studies. Current areas of interest include:

  • Antimicrobial and clinical pharmacokinetics/pharmacodynamics (PK/PD)
  • Precision dosing and model-informed precision dosing
  • Therapeutic drug monitoring (TDM)
  • Critical care and intensive care medicine
  • Clinical trials and multicentre observational studies
  • Personalised medicine and dose individualisation

Paul leads the international DALI-2 study and contributes to several national and international collaborative research programs investigating medicine dosing and outcomes in critically ill patients. His goal is to generate evidence that supports safer and more effective use of medicines in clinical practice and improves outcomes for patients with severe infections and critical illness.

Paul welcomes collaboration with clinicians, researchers, and students interested in critical care pharmacotherapy, precision dosing, and antimicrobial optimisation.

Research interests

  • Antimicrobial Optimisation in Critical Illness

    Research focused on improving the effectiveness and safety of antimicrobial therapy in critically ill patients. This work investigates antibiotic exposure, target attainment, dosing strategies, and antimicrobial stewardship to optimise patient outcomes and reduce the emergence of antimicrobial resistance.

  • Precision Dosing and Personalised Medicine

    Developing and evaluating approaches to individualise medicine dosing using patient-specific factors, therapeutic drug monitoring, pharmacokinetic/pharmacodynamic (PK/PD) principles, and model-informed precision dosing. The goal is to improve treatment effectiveness while minimising toxicity.

  • Clinical Pharmacokinetics and Pharmacodynamics

    Investigating how critical illness alters the disposition and effects of medicines. Research focuses on understanding sources of pharmacokinetic variability and applying PK/PD principles to support evidence-based dosing recommendations for critically ill patients.

  • Therapeutic Drug Monitoring

    Evaluating the role of therapeutic drug monitoring in improving medicine exposure and clinical outcomes. This research explores the implementation and optimisation of TDM strategies to support safer and more effective dosing in clinical practice.

  • Critical Care Pharmacotherapy

    Research examining the use of medicines in critically ill patients, including anti-infectives, sedation, and analgesia therapies. Current work aims to improve dosing strategies and clinical outcomes through observational studies, clinical trials, and pharmacometric research.

  • International Clinical Dosing Studies

    Leading and contributing to national and multinational studies investigating medicine dosing and outcomes in critically ill patients. This work generates evidence to inform clinical guidelines, support precision dosing, and improve medicine use across diverse healthcare settings.

Research impacts

Paul's research focuses on improving the safety and effectiveness of medicine use in critically ill patients. By investigating how critical illness alters drug exposure and response, his work supports more precise and individualised dosing strategies that can improve patient outcomes and reduce treatment failure.

His research has contributed to the development and evaluation of antimicrobial dosing strategies, therapeutic drug monitoring practices, and precision dosing approaches used in intensive care. Dosing guidance arising from this work has been incorporated into local clinical practice and electronic prescribing systems, supporting evidence-based decision-making at the bedside.

Paul contributes to national and international collaborations that generate evidence to improve medicine dosing in critically ill patients. Research to which he has contributed has informed international clinical practice guidelines, including the Surviving Sepsis Campaign Guidelines, helping translate research findings into recommendations used by clinicians worldwide.

He is also a pharmacist representative on the Therapeutic Guidelines Antibiotic expert group for hospital infections, contributing to the development of national guidance used by healthcare professionals across Australia.

Through research leadership, clinical engagement, and multidisciplinary collaboration, Paul aims to bridge the gap between pharmacology research and patient care, ensuring that advances in precision dosing translate into safer and more effective treatment for patients with severe infections and critical illness.
